BOOKING POLICY

• ALL CALLS ARE ON CENTRAL STANDARD TIME ZONE.

• ALL CALLS ARE AUDIO BY PHONE OR FACETIME (A ZOOM CALL CAN BE ARRANGED IF NEEDED)

• RECORDING OF ANY CALLS IS PROHIBITED.

• No refunds under any circumstances.

• For all calls you’ll receive an appointment reminder 24 hours in advance. For Zoom calls; a link will be sent in an email you will receive from support@nursenguyen.com

• No call/ no shows will not be refunded, and will be prohibited from booking any services in the future.

• If you need to reschedule, it MUST be 48 hours in advance.

• You have a FIVE minute grace period after I begin the call, once the grace period passes, I will have to end the call.

• You may only reschedule ONCE, or you will have to rebook

PAYMENT

• I accept payments at the service link provided.

• If you experience issues with payment link, I’ll send you an invoice that can be paid using a debit or credit card.

• Pre-Payment is required to book your session.

• No refunds under any circumstances!

CONSULTATIONS/ SURGERY PLAN ADVISING CALLS

• After booking, you’ll receive an email with a questionnaire form to fill out that will collect general information (health questionnaire, surgeons of interest, and pictures that will be used for the consultation)

• I will send a Liability and Waiver Release form for you to sign digitally via Adobe Sign. It MUST be reviewed and signed before the call, or we will not be able to move forward. You have a 72 hour time frame before the date of your call to submit the necessary information I need to review. If it isn’t submitted, you’ll have to reschedule your call.

POST OP GUIDANCE CALLS

• I strongly suggest scheduling your call once you’ve made it home from surgery. This is for your comfortability.

• If you’re more than 8-9 months+, you MUST send a picture of yourself before booking to ensure that a call will still be beneficial for you.

• Please prepare questions

CANCELLATION

• If you need to reschedule, it MUST be 48 hours in advance.

• No call/ no shows will NOT be refunded, and will be prohibited from booking any services in the future.

*I reserve all right to refuse and/ or cancel all services if at any moment I deem a surgeon you’ve selected unsafe. I also reserve the right to cancel all services if vulnerable abuse is endured or if constant contact / harassment takes place.*
